Create a new Custom Form

  1. Head over to the Custom Forms dashboard

2. Click on 'New Form' at the bottom of the screen

The form builder screen consists of three sections. The left side of the screen contains all the fields and components that you can drag and drop onto the form canvas. The rest of the screen is the form canvas, which is built by dragging and dropping fields and components from the left. The third section only appears when a field or component is selected from the form canvas. This section shows up on the right side of the screen.


There are two types of fields that can be added to form, fields that are linked to CRM fields and general fields that are only liked to the form itself. Please see below for details on these field types.

All fields can have a custom label, or question, added to in place of the general name of the field. 

  1. Click on the form field from the form canvas to bring up the component details view

  2. Fill out the 'Label' field with whatever you want the form to display. For example, the field name may be Affiliate, but the form itself should ask 'What affiliations or groups do you maintain   membership with?'. You will see the field on the form canvas updating with the new label as you   type it.

  3. Click 'Save Form'


The fields listed in the sections entitled Standard Fields, Custom Fields, and Practice Area Custom Fields are all fields that are linked to CRM fields. You can also identify these fields because they have the name of the CRM field on them.

When these fields are included on a form, the answers submitted will be set as the value for that field on the CRM record. The data in those fields will then be available for use everywhere in the system where CRM fields are used (i.e. automations, e-signature docs, etc).


The other field type, general, is to be used for questions on a form who's answers are not necessary to save on the CRM record itself.

General fields are accessed by dragging over the General Field field component from the left side, under the heading Advanced

This general field can be dragged over to the form canvas the same as any other component.

Once a general field component has been dropped onto the form canvas, you can click on that field on the form canvas to access its properties and set the field up.

1. Fill out the custom label for the field to indicate to the person what you are asking them (i.e. Please tell us about yourself).

2. Chose what type of field you need this general field to be. You can select for many different field types including true/false, number, money, and picklist (which you can create the options for right here on the fly).


Lawmatics has the ability to show fields to users depending on their answers to other questions on the same form! These conditional fields are easy to set up.

A field can only be made conditional, meaning hidden or shown, on another field if that other field is either true/false type or picklist type.

To make a field hidden/shown based on the answer to another field, please follow these steps

  1. Click on the field that you would like be hidden or shown to display the details view

  2. Turn on the 'Conditional Logic' toggle switch

  3. Select the field which will control whether this field is hidden or not

  4. Select the answer to the control field that will cause the field to SHOW. For example, in the   above image we are saying that we only want the 'Tell us why you love us so much!' field to be   shown if the 'Do you love Lawmatics?' field is TRUE. If it is false the 'Tell us why you love us so   much!' field will not be displayed on the form.

  5. Click 'Save Form'
